Earn $38/hr (including full hourly wage for drive time)!
We specialize in providing in-home care for former Nuclear Weapons Workers suffering from chronic and terminal illnesses due to their work environment. As an RN Care Coordinator, you will conduct essential in-home nursing visits, typically lasting 1-2 hours each week.
In this rewarding role, you will perform physical assessments, monitor vital signs, ensure home safety with medical supplies, and manage weekly medication planners. You'll also oversee both skilled and non-skilled caregivers assisting your clients at home.
This position allows you to build meaningful one-on-one relationships with your patients while enhancing their health and quality of life. With the freedom to set your own schedule, you'll have the flexibility to pursue education, travel, volunteer, or spend quality time with family and friends.
Qualifications:
Graduation from an accredited school of professional registered nursing
BSN preferred
Current, unrestricted RN license in the state of practice
At least two (2) years of nursing experience, including one (1) year in home care or a related field
Preferred: one (1) year of supervisory and/or case management experience
Current CPR certification
Key Responsibilities:
Deliver direct care and manage assigned client cases
Collaborate with the regional director and team to enhance service delivery
Plan, implement, and assess the care provided
Manage client care conferences as needed
Serve as a clinical resource and support for client home care needs
Work with staffing coordinators to ensure adequate personnel for client needs
Adhere to nursing guidelines as per Agency policies
Participate in hiring, training, and evaluating clinical staff
Review clinical documentation for accuracy
Engage in all Agency performance improvement initiatives
Conduct supervisory visits to support problem resolution
Analyze reports for opportunities for performance improvement
Participate in marketing activities to promote Agency programs
Be available during operating hours for clinical support
