JOB DUTIES/RESPONSIBILITES:
• Conducts psychosocial assessments to document history and identify preliminary issues for treatment focus by interviewing the patient and their family members.
• Coordinates care of assigned patients throughout their stay at SM and participates in patient rounds to plan and evaluate individualized safe and timely patient discharge needs.
• Coordinates referrals for these services and utilizes interpersonal skills to maximize excellence in customer service, safe patient care, and professionalism.
• Develops and coordinates an individualized discharge plan for the patient by utilizing team and chart information to determine the patient's aftercare needs.
• Works with Discharge Planner to develop, implement and document a discharge plan established with the patient per JCAHO standards and with attention being focused on progressing toward discharge.
• Attend Treatment Team Meetings as determined by the Director of Clinical Services
• Demonstrates current knowledge about full range of discharge options/community resources and utilizes them in the patient's discharge plan.
• Identifies and assesses family or community resources such as, group homes, or mental health practitioners to meet patient's aftercare needs.
• Reports to community/state agencies any patient issues required and represents facility in any legal proceedings.
• Provides group and family process-oriented therapies as well as conducts educational and other didactic groups for patients and family members using various professional treatment modalities.
Skills:
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
• Experience with age-specific inpatient setting preferred
• Experience in taking psychosocial history and case management
• Knowledge of developmental psychology of adolescent and adults and have the capacity to identify pertinent clinical information
• Possess knowledge of clinical issues related to mental health patients and their discharge planning
• New hires are required to attend 1 full week of New Employee Orientation on day shift, normally 8:
00 am - 4:
30 pm Monday through Friday
• Must successfully pass background check (includes criminal history, Child Abuse and Neglect Registry, and ABI/FBI fingerprint check) and drug screen
Education:
QUALITFICATIONS:
• Master's degree in social work
• Current licensure in the State of Nevada; also acceptable licensure:
CSW-I, LCSW
