Work for a leading provider of integrated medical services in various settings including skilled nursing facilities, assisted living, and private homes.
Responsibilities:
Provide evidence-based medical care by assessing and addressing patient medical needs.
Treat chronic conditions and urgent episodic illnesses, as well as minor injuries.
Order and interpret lab tests and diagnostic procedures as necessary.
Collaborate with healthcare professionals and caregivers for coordinated care plans.
Refer patients to specialists when required and ensure updated medical records.
Qualifications:
Active NP license and National Board Certification.
DEA licensure and a Master's degree in Nursing (MSN).
Minimum five years of nursing experience with at least two years as a nurse practitioner preferred.
Strong communication and time management skills.
Proficient with medical equipment and EMR systems.
Valid driver's license with automobile insurance.
Ability to perform moderate physical activity, including lifting up to 25 lbs.
