Job Summary
Memorial Hermann seeks a Home Infusion Pharmacist to join our Katy Specialty Pharmacy team.

Schedule:
Monday through Friday, Days, plus on-call every 5 weeks.

Home Infusion experience is highly desired.
Inpatient hospital experience a plus.

Must have IV Certification.

The Pharmacist will work according to legal and ethical guidelines to ensure the correct and safe supply of prescriptions and supplies to Home Infusion patients.
The role provides patient centered care including, but not limited to:
oversight, monitoring, preparation, dispensing, and proper documentation of patient medication therapy.
The role also ensures compliance with Federal, State and local laws and regulations as well as proper medication storage and documentation.
Typically reports to the Home Infusion Pharmacy Manager.



MINIMUM QUALIFICATIONS
Education :
Graduate of an accredited school of pharmacy
Licenses/Certifications:


Current State of Texas license/permit to practice pharmacy
Experience / Knowledge / Skills:

Minimum two (2) years of experience in clinical area of practice
Knowledge of home infusion medications
Familiarity with health care reimbursement
Experience in pharmaceutical care related to intravenous homecare patients preferred
PRINCIPAL ACCOUNTABILITIES
Prepares labels and dispenses medications for patients in accordance with physicians' prescriptions; ensures compliance with Federal, State and local laws and regulations pertaining to the dispensing of drugs, including narcotics and the maintenance or required records.
Answers questions and provides information to pharmacy customers on drug interactions, side effects, dosage, administration and medication storage.
Receives and reviews all prescriptions for accuracy and clinical appropriateness.
Ensures that complete and accurate business and clinical records are maintained.
Supervises the activities of the pharmacy technical staff and mentors or trains pharmacists, pharmacy students or residents.
Maintains medication profiles, reviews profiles for drug related problems, and provides consultation on drug research.
Maintains the scheduling and preparation of routine and new prescription orders.
Initiates reports of medication errors or adverse drug interactions.
Works closely with the Director and Manager of Pharmacy to coordinate daily operations and the implementations of new and existing programs.
Complies with all regulatory requirements, including HIPAA/HITECH.
Responsible for provision of all facets of pharmaceutical care related to intravenous homecare patients.
Maintains knowledge for safely and correctly, compounding sterile medications and infusions for the home, outpatient clinics, skilled nursing facilities and physician/clinic environments.
Maintains the knowledge to initiate infusion therapies with successful on-going evaluation of care and completion of required documentation relative to processing therapies, both fiscal and legal.
Responsible for understanding of, compliance with and participation in Quality Assurance, Performance Improvement and accreditation guidelines ensuring achievement of excellence in quality patient care.
Assist with the referral process including but not limited to:
provision of medical information to patients and caregivers relative to home infusion therapy; assistance in the teaching of patients prior to or after discharge to home on medication administration; and coordination of deliveries with nursing and support services
Be actively involved in the education and in servicing of licensed health care professionals who perform services relative to the therapies provided by the home infusion pharmacy.
Maintain working knowledge of all equipment and supplies necessary to administer therapies in the home or outpatient setting.
Participate in inventory control, ordering, warehouse organization, packing/shipping, equipment maintenance and hazardous waste control.
Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
