SUMMARY

The Vascular Coordinator plays a key role in supporting the day-to-day operations of the vascular surgery clinic. This position provides administrative, scheduling, and clerical support to physicians, nurses, and other clinical staff, ensuring seamless coordination of patient care and procedures. The coordinator acts as a liaison between patients, providers, and departments, facilitating communication and ensuring a high standard of patient service. The coordinator also actively monitors patient follow-up, ensuring continuity of care and adherence to clinical guidelines and best practices.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Coordinate and schedule vascular diagnostic tests, procedures, surgeries, and follow-up appointments.
Track patient appointments and follow-up intervals to ensure patients return for necessary post-operative and surveillance visits (e.g., aneurysm repair, carotid disease, PAD).
Develop and maintain tracking systems or databases to monitor patient follow-up timelines.
Contact patients via phone, mail, or electronic portals to remind them of upcoming appointments or to reschedule missed visits.
Collaborate with clinical staff to ensure timely communication with patients regarding test results, medication changes, or further care instructions.
Identify and flag patients at risk of being lost to follow-up, and implement re-engagement strategies.
Work with providers to develop individualized patient follow-up plans in alignment with clinical best practices and national guidelines.
Support implementation and adherence to clinical care pathways, quality measures, and vascular-specific best practices.
Participate in quality improvement initiatives by collecting and reporting data related to patient outcomes, follow-up adherence, and care gaps.
Monitor and promote patient compliance with lifestyle modification programs, medication adherence, and post-operative care plans.
Provide education materials or coordinate with clinical educators to support informed patient decisions and compliance.
To ensure coordinated care, liaisons between patients, physicians, imaging centers, OR staff, and external referring providers.
Facilitate the scheduling and documentation of multidisciplinary vascular team meetings.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

High School diploma or equivalent required.
An associate's or Bachelor's degree in Health Administration, Nursing, or a related field is preferred.
2+ years of experience in a healthcare administrative role, preferably in vascular, surgical, or cardiology settings.
Familiarity with Epic electronic health record systems.
SKILLS :

Strong organizational and multitasking skills with keen attention to detail.
Excellent verbal and written communication skills.
Ability to work independently and collaboratively with clinical teams.
Proficiency in Microsoft Office and patient tracking software.
Understanding of medical terminology and vascular care protocols.
PREFERRED

Background in care coordination, case management, or patient navigation.
Certification in medical office administration or health unit coordination.
Bilingual abilities to support diverse patient populations.

SUPERVISORY RESPONSIBILITIES

This job has no supervisory responsibilities.
