About You:
Doctor of Dental Surgery (DDS) or Doctor of Medicine in Dentistry (DMD) from an accredited dental school; Or currently enrolled in an accredited dental school for DDS or DMD.
Current and valid dental license to practice or ability to obtain one.
DEA certification required.
CPR/BLS certification (or ability to obtain).
