A dedicated and conscientious care worker with experience in the health care sector.
Provides essential assistance with the elderly and those unable to carry out everyday tasks independently.

Responsibilities

•             Assist clients with personal care tasks, such as washing, dressing, eating and toileting.

•             Perform light housekeeping tasks

•             Assist in the safe lifting, transferring, re-positioning and movement of clients

•             Accompany clients to medical appointments

•             Observe, monitor and record clients’ physical and emotional well-being, and promptly report any changes to senior staff.

•             Encourage clients to participate in social and recreational activities.

•             Maintaining patient confidentiality and respecting the rights of patients.

•             Helping the customer to plan what to eat and drink

•             Gentle encouragement and help to eat and drink well

•             Shopping, preparing and serving food and drinks

•             Encouraging, reminding; assisting and giving medicines

•             Ordering and collecting prescriptions

•             Act as Support worker to clients, ensuring their needs are met.
To participate in the formulation and implementation of individual Support plans.

•             Assisting with the administration of medication with appropriate training

•             Participate in assessments and reviews and contribute to good written and verbal communications while respecting client confidentiality.

•             Observe clients wishes with regard to patterns of daily living.
Organise and participate in activities and outings involving the client in planning where possible.

•             Act as escort where necessary.

Requirements:

•             High school diploma preferred, but not necessary.

•             Training in safe patient handling & movement desirable (Free Mandatory Training Offered).

               Previous rehab experience required.

•             Ability to communicate well, verbally and written.

•             Ability to understand and follow policies and procedures.

•             Ability to work as part of a team.

•             Ability to use initiative.

•             A willingness to learn and adapt to change.

              Driver essential.
