Advance Practice Nurse -

Palliative Pain and Symptom Management Consultant

1 - Part Time, Permanent (28 hours/week)

Manager, Community Health Teams

Hospice Palliative Care Team

Purpose:

The Palliative Pain and Symptom Management Consultant (PPSMC) plays a vital role in strengthening the capacity of front-line service providers and enhancing palliative care delivery across the healthcare system. This position focuses on creating a supportive environment through education, mentorship, and resource development.

Key areas of responsibility:

  • Provide expert consultation to healthcare professionals on palliative pain and symptom management, driving improvements in patient care delivery.
  • Enhance the skills of frontline service providers in delivering palliative care through education and mentoring, using effective adult education principles.
  • Develops and delivers seminars, in-service training, and workshops on subjects pertinent to the goals of the organization and the professional development needs of front-line providers.
  • Promotes standardized education in palliative care to increase the number of professionals and volunteers receiving advanced training and certification in end-of-life care. Courses offered include Fundamentals of Hospice Palliative Care, Comprehensive Advanced Palliative Care Education (CAPCE) and Advanced Practice Palliative Skills (APPS)
  • Develop and facilitate educational programs for front-line providers, including workshops, seminars, and one-on-one mentorship sessions.
  • Identify and curate palliative care resources, creating easy access for service providers to enhance their practice.
  • Foster relationships with healthcare teams, organizations, and community partners to create connections and improve coordination of palliative care services.
  • Act as an expert advisor in strategic planning initiatives, contributing insights and recommendations that shape healthcare system development in palliative care.
  • Provide leadership and guidance at palliative care rounds and case conferences.
  • Collaborate and consult with the Palliative Pain and Symptom Management Consultants across Ontario, particularly those in the Central East and Toronto.
  • Contribute to data collection and quality management initiatives and documentation.
  • Work collaboratively and effectively with the interdisciplinary team at SCHC.
  • Involvement in policy and procedure development and implementation.
  • Participate in training workshops to enhance skills and knowledge.
  • Participate in the accreditation process, implementation of strategic plans.
  • Perform other duties as assigned that are reasonable within the scope of the job.

Educational and/or Professional Qualifications:

  • Registration and in good standing with the College of Nurses of Ontario.
  • Undergraduate degree in nursing from an accredited University/College,
  • Master's preferred.
  • CNA Certification in Hospice Palliative Care Nursing an asset.
  • Membership with RNAO an asset.
  • Holds current Basic Life Support for Healthcare Providers certificate.
  • ICU and/or emergency department experience an asset.

Level of Experience:

  • Minimum five years of palliative care nursing experience.
  • Current clinical knowledge, experience and expertise in palliative care, particularly with palliative pain and symptom management.
  • Demonstrated knowledge of, and expertise in, facilitating adult education for a variety of health disciplines.
  • Demonstrated knowledge of health care system (institutional and community) as it relates to palliative care delivery.

Skills and Attributes:

  • Highly developed interpersonal, communication, organization, coordination, conflict management, and time management skills.
  • Experience working with Zoom, Teams and Microsoft office and it's delivery methods.
  • Experience in program development and coordination.
  • Demonstrated commitment to interdisciplinary care.
  • Demonstrated skill in problem solving and critical thinking.
  • Demonstrated interpersonal, communication and teamwork skills.
  • Demonstrated commitment to continuous learning and professional development.
  • Excellent computer skills including Outlook, Word, Excel and PowerPoint.
  • Oral and/or written fluency in other languages relevant to providing service in the role would be considered an asset.
  • Sensitivity and awareness of culturally and racially, economically and socially diverse communities.
  • Willingness to preceptor students from accredited academic facilities.

Significant Working Conditions:

  • Ability to work flexible hours including evenings and weekends to meet the needs of the program.
  • Ability and willingness to travel across Scarborough to a variety of care settings including client's home, long term care facilities, hospitals, and other community settings.
  • Must have access to own vehicle.

Remuneration:
$40. 76 - $47. 55
