Department/Unit Summary
The RN Navigator demonstrates performance consistent with the mission, vision and values of the organization.
They remain flexible to changing systems - is expected to demonstrate quality, effectiveness in work habits, clinical practices, and treats staff, physicians, patients and families with consideration and respect.
As allowed by their licensure, prepares patients and assists during examinations, treatment, procedures, patient navigation, clinical support, limited surgical support function and/or diagnostic testing.
Maintain current knowledge base on complications associated with surgery for vascular patients.
Assist physicians in the health maintenance of patients in a complex disease state.
Patient navigation and care coordination for vascular clinic patients.
Assist doctors in performing physical tests and prescribed tests.
Help to conduct and interpret laboratory test results and help to interpret results.
Work with doctors and other departments whenever a patient is undergoing surgery.
Monitor/manage, screen telephone requests for assistance by urgency of the existing patient base.
Interview and assess patients for medical interventions in the office and on the telephone.
Initiate medical interventions based on a symptom-focused protocol or guideline.
Educate and monitor patient compliance regarding medical interventions.
Monitor the psychosocial and emotional needs of patients for outside referrals and additional care.
Assist the physician with the medication management of the existing patients.
Support medical assistants, perform quality improvement audits and tasks within the clinic.
Assist in the pursuit of specialty drugs and/or prior authorizations on behalf of the patient.
Prepare and maintain accurate medical records.

Minimum of 2 years of nursing experience, preferably in cardiac or vascular care
