MAIN FUNCTION: Assist registrants with all activities of daily living, which includes bathing, dressing, nail care, toileting and any other needs the registrant may require. Assistance must also be provided in a broad program of activities.
RESPONSIBILITIES: Must be available to periodically work more than eight (8) hours per day, or more than forty (40) hours per week.
Receive verbal assignment from the Adult Day Health Care Director or Nurse and plan day accordingly. Post registrant rehabilitation schedule, making sure appointments are kept.
Checks vital signs, including temperatures every month, blood pressure and pulse every day or as directed by the Day Care Director. If a weight, or any vital sign is unusual, ask the nurse to re-check, then document appropriately. Weights are done weekly or monthly as ordered.
Perform assigned duties such as: answering the phone, assisting and directing relatives, visitors and volunteers any other assignment the Nurse/Adult Day Health Care Director may give.
Follow the policy and procedures the Adult Day Health Care Director may give.
Perform personal services to the registrant such as: reading to them, helping them write letters, socializing with them as often as possible, assisting them with activities when necessary.
Observe registrant behavior and report any changes or unusual findings to the nurse in charge.
Assist with serving and cleaning up after meals.
Assist registrants in preparing for doctor's visits, religious services, rehabilitation services, diagnostic services and social events. Make sure that registrants attend Rehab as ordered and document this.
Take care of supplies and equipment. Utilize proper disinfecting procedures with cleaning equipment.
Keep incontinent registrants clean and dry, changing clothing as necessary. Give immediate peri-care and encourage good hygiene and grooming practices. Assist with C-tub as ordered by physician. Make sure that the registrant receives the appropriate number of C-tubs. Notify the nurse/Adult Day Health Care Director if the registrant does not receive the prescribed number/week.
Supervise and assist registrants during meals checking to see that they eat, and if not, report to program director.
Treat all registrants' information with confidentiality.
Attend at least six hours of in-service education every six months to maintain CNA certification.
Clean shower and tub after each registrant use, maintaining policies and procedures involved with infection control.
Assist with emergencies and agitated registrants as directed.
Assist in providing a comprehensive program of activities, tailored to meet the needs of each individual registrant to promote optimum health, independence and self care.
Assist in developing specific activities for the program.
Maintain standard records of work performed and registrant participation throughout the day.
Assist registrants with transferring and ambulation when indicated.
Perform other duties as directed by the program director or the nurse in charge.
Other related duties that may have become necessary or as directed by the supervisor, department head and/or administration.
QUALIFICATIONS:
Knowledge of principles and techniques involved in dealing with the physical, mental and emotional health of the disabled, elderly and functionally impaired adult.
Preferred to have skill in assisting activities staff and implementing activities.
Preferred to have skill in personally leading groups and in using equipment such as movie and slide projectors, video players and record players.
High School Diploma or G.E.D. required.
Current New York State Nurse certification required.
CPR certification required.
Previous experience preferred, on-the-job training provided.
Sense of responsibility, moral integrity and understanding is required.
Sincere desire to work with the disabled and elderly.
Good mental and physical health required.
Pleasant, cheerful, tactful with residents, relatives and visitors, and with other staff members.
Ability to remain calm in emergencies.
